New Patient Registration and GP Allocation

register_now_blue_signIf you are not currently a patient of the practice, you live within our practice boundary and you wish to register, you simply need to call in to the surgery to pick up registration forms. You will also be asked for information of your permanent address and confirmation of identity.

Our receptionists will provide you with all the relevant forms that you need to complete. Separate forms must be completed for each member of your family. The receptionist will give you the name of your allocated GP which is usually offered in rotation.  This may be a male or female GP.   If you have a specific requirement, please inform the receptionist, and your request will be considered.

If you have recently moved into the area from abroad or do not intend to reside here long term you may be registered as a temporary patient. Many, but not all countries, have reciprocal arrangements with the NHS.  This means that sometimes a charge is applicable - this will be discussed with you at registration.

Everyone is welcome in general practice.  Patients are not legally required to show proof of ID, address, immigration status or an NHS number in order to receive care or see a GP.  This just helps our administrative processes.  Medical treatment is available from the date of registration.
